The Day of Your Appointment
On the day of your appointment, please arrive 15 minutes early. This gives us enough time to finish up paperwork that needs to be completed and remain on time for your appointment.
If you're a first time patient, please bring a valid driver's license or form of identification. To save time, you can also get started on filling out paperwork.
